Terms and Conditions: Decoder Report

What is involved?

You are purchasing a twelve-month, non-recurring Decoder Report subscription cycle.

Within a subscription cycle, you will receive three online health questionnaires to complete. These will be issued upon payment and six and twelve months later. Data collected from each survey is transformed into a customised Decoder report. Therefore, it’s important you complete the surveys honestly and to the best of your ability to ensure the most accurate results.

Each Decoder report is securely transmitted to your nominated doctor within 24 hours of survey completion. Your doctor reviews and discusses the Decoder report with you at your next booked appointment and can release a copy to you and/or your nominated health professionals as appropriate.

You will also be requested to provide intermittent feedback surveys to help us improve our service.

What do I gain from my doctor receiving these reports?

The Decoder report summarises your health data to help your doctor better understand your concerns and unique pain challenges. The serial reports also allow you and your doctor to track your progress.

The Decoder report is not a substitute for personalised medical advice and should not be interpreted by anyone (including yourself) without approval from the referring doctor.

How much do the reports cost?

The total cost of an annual subscription to three Decoder reports is $50 (incl. GST).

Reports cannot be purchased separately.

All purchases are non-refundable and non-transferable.

Unique Subscription Tokens (UST) are redeemable codes issued by health entities to improve access to Decoder reporting. USTs offset the annual subscription cost entirely and cannot be redeemed more than once or combined with other discount codes.

What is expected of me?

Complete all surveys honestly and within seven days of request.

Ensure you have a doctor’s appointment booked after each survey to discuss your results.

Do not modify, reproduce, or distribute any RUBIX HEALTH content.  All information on the RUBIX HEALTH website, questionnaires and Decoder reports is copyright protected.

What if I need to cancel?

You can cancel anytime by contacting RUBIX HEALTH staff by phone or email. Your doctor/s will be automatically notified. If you wish to re-engage RUBIX Health for further Decoder reports, a new annual cycle must be purchased.

What if I have a question or complaint?

Please direct all queries and complaints to RUBIX HEALTH staff. Your concern will be acknowledged within two business days, and you will be contacted directly to resolve the issue.

What measures are taken to protect my privacy?

Your data is securely handled and stored in accordance with the RUBIX HEALTH Privacy Policy.  This policy aligns with the Australian Privacy Act and ISO27001 standards.

RUBIX HEALTH will only use your data to communicate with you, generate your Decoder reports and assess the program’s success.

Please see the Privacy Policy for complete details on data handling, privacy and security.

Who receives a copy of my Decoder Report?

The Decoder report is complex and requires medical interpretation. For this reason, we do not send Decoder reports directly to you or any non-doctor health practitioner, even at your request. You can obtain a copy of the Decoder report from your doctor once it has been reviewed and discussed with you.

RUBIX HEALTH will share the Decoder Report with the doctor you nominate.

Please note:

  • If a doctor or medical centre provides you with a UST, the doctor will receive a copy of the report if you redeem that UST. The doctor’s details will appear in the list below. You are not obligated to use the UST and can remove the token if you do not wish this to occur.
  • If a third party other than a doctor or a medical centre provides you with a UST,  the entity will not receive a copy of your Decoder if you redeem the UST.
